Ms. Jill Kovalcik Weaver called the meeting to order at 11:06 a.m.
There were no additions.
Mr. Larry Kauffman motioned to approve the agenda and the minutes. Ms. Leslie Kelly seconded the motion. All were in favor.
Ms. Kovalcik Weaver reported the survey was approved at the November Full Council Meeting.
Members discussed that the survey would run from December 1, 2009 through September 30, 2010. The survey will be sent via the survey tool, survey monkey, the weekly update and through other liaisons of the council. Members will look at the data quarterly. Ms. Nancy Massey will provide the committee with those reports March 31, 2010, June 30, 2010, September 30, 2010 and annual report. Ms. Massey will put the survey on the home page with an icon so the user will see the survey quickly when they access the home page. Members discussed tracking geographic location. These surveys will not be handed out at the State Plan attachment meetings. Information gathered from the annual report will be given to the State Plan committee for use in the State Plan attachment. Members discussed adding a historical perspective to show why a consumer should fill out the survey.
